Genomic DNA or blood with EDTA (chorionic downiness if prenatal test)

How do I send the sample?

Enter a duly numbered and identified eppendorf tube (preferably low-bind) with genomic DNA or 3ml tube of blood with EDTA (mauve tube) in a padded envelope or box closed.

Send the package by regular mail (so you do not arrive on a weekend) to the following address:

What documents do I need to send the sample?

• Request Sheet (attached).
• Patient clinic (short summary) History, essential for proper development of the final report.
• The declaration of the existence of informed consent.
• Commitment to pay the hospital or Petitioner (may be an individual) and data for billing and shipping of the report: CIF, registered office, telephone, email.

What do I need for the family study of one or more variants detected in the index case?

• Sample DNA or EDTA blood of each of the family members to study (affections and not subject) properly identified.
• Request Sheet (attached) in which he made explicit: family study XXXX XXXX XXXX patient for the following variants detected in the index case.
• The declaration of the existence of informed consent
• Commitment to pay the hospital or Petitioner (may be an individual) and data for billing and shipping of the report: CIF, registered office, telephone, email.

• Prenatal testing must be requested at least 1 month before the sample shipment.
• The sample must be DNA extracted from chorionic villi.
